The Child Disability Register (CDR) is a confidential, secure computer database which holds information about children and young people with any significant disability or life altering health condition, aged 0–19.

To help with their work in Sheffield, Health, Education and Social Services have joined together to consolidate all information they hold on children, young people and their disabilities. It is a city wide project backed by the NHS and the Government.

The CDR enables us jointly to:

  • Collect and supply information for planning – for future years and the immediate future – from Schools and Transport to Holiday Schemes and Respite Care. This makes sure children and their families are included in planning services.
  • Provide statistical information to Government.
  • Ask children, young people and their families what they want. Without consulting parents, carers and children about what questions they would like answered, the CDR could not work.
  • Circulate relevant information, ideas and news via a regular Newsletter.
  • Ensure children and their families know what is going on in the city, and how to reach and obtain these services.

How does the CDR work?

  • The CDR is based at the Sheffield Children’s NHS Trust Ryegate Children’s Centre and managed by a Co-ordinator, Martina Capaldi.
  • Enrolment is voluntary and all personal information on the CDR is confidential.
  • Questionnaires will be completed by parents or together with a professional who knows the child well, or the CDR Co-ordinator.
  • All parents and families will be given a copy of their information held on the CDR, to use as they wish.
  • Information held on the CDR will be periodically updated, via contacting the family – therefore as the child/young person matures vital changes will not be missed.
  • Families will receive a regular Newsletter full of news, ideas and information plus a membership card.
